Help from the CTR Oversight Panel
This panel is for autistic people and people with a learning disability who:
- may have had a recent CTR (for example, within the last year)
- despite having support from professionals and agencies, things are not getting better
- despite having CTRs, their care and treatment plans are not working for them
- are having difficulties despite agencies delivering the right care.
- are having difficulties because of problems within the system across different professional teams and agencies
- The panel meets once every 6 weeks.
- The panel are professionals at a senior level in health and social care and people with lived experience.
- The panel works to check people are safe, concerns are sorted out and everyone learns to make your care better.
- They may be able to take things to a higher level and support those working with you to get the help that you need.
- The panel is also concerned when problems are caused by different services and professionals in the system not working well together.
- If people may have been left alone for long periods of time (seclusion) Why? How often?
- When complaints have not been resolved or things have not improved
- How safeguarding referrals are being managed
- When there are long delays in getting the right support or in discharge planning
- Concerns that have been raised about safety, care, support and treatment
- When people in hospital are declining offerings of a CTR
- Issues that may need to be raised to a higher level in health and social care
- Other concerns
The panel will not look at
- Urgent or emergency issues that have only just happened
- Issues that are already being looked at- like a complaint or safeguarding alert
- You have read the information about the Oversight Panel.
- You are happy for the panel to be involved
- You are happy for the panel to read reports about you so they understand your situation better.
- Your personal information is private. Only the panel members and people involved in your care and/or treatment will be able to see it.
- By clicking submit on the online form you are giving permission for the panel to consider your case and information about you.
- If you are sending this form on behalf of a family member or someone you professionally support, clicking submit means that you have the person’s permission for the panel to consider the person’s case and information written about them.
- You can tell us you want to change your mind and withdraw consent to your information being shared with the panel at any time. To do this you need to telephone 01634 335095. This is a messaging service so you will need to leave a message and include your name, telephone number or email in case the panel need to get in touch with you.
There is one referral form. This form is for both people (and families or advocates) and for paid staff.
